Job Description Social Worker - Hospital Based Schedule 8:00 AM - 4:30 PM, Monday to Friday
Assignment Details Guaranteed Hours: 40 Weekly
Contract Length: 12 Weeks
Requirements - Minimum of 2 years of experience within specialty
- AHA BLS Certification
- GA LCSW License
- Acute hospital experience
Responsibilities - Provide assessment and intervention services to patients and families.
- Collaborate with interdisciplinary teams for coordinated patient care.
- Facilitate discharge planning and continuity of care.
- Utilize evidence-based practices in social work interventions.
- Document patient interactions and maintain accurate records.
