Social Services - LMSW

Schedule

Assignment Length: 26 or 13 Weeks
Shift: 5x8s or 4x10s

Assignment Details

Guaranteed Hours: 72 Hours per 2 weeks

Requirements

• Able to apply for a temporary LMSW license in the state
• 2 years of acute care experience
• Master's degree from a CSWE accredited school of social work
• Skills as required by the unit
• Weekend availability - every other
• On-call is not required

Responsibilities

• Provide psychosocial/emotional support to patients and families
• Complete psychosocial assessments and develop appropriate discharge plans
• Assist with resource navigation and coordination of care
• Screen for suicidality and engage in safety planning as necessary
• Coordinate with external entities as required
• Work collaboratively with RN Case Managers and other care team members
